UPDATE: This evening, the Jindal administration announced that it was suspending — through June 30 — its announced plan to cease funding of Medicaid hospice services. As the New Orleans Times-Picayune reports:
“…. on Wednesday, the administration reversed course. The program will be maintained for the rest of the state’s fiscal year, which ends June 30, with the use of “temporary bridge funding,” according to a news release from the Department of Health and Hospitals. The release referred to that money as grant funding but did not specify where it came from or how reallocating it to hospice care would affect other state programs.”
The longer-term fate of the Medicaid hospice program is uncertain.

How do conservatives propose to cut Medicare and Medicaid spending?
Gov. Bobby Jindal, oft-mentioned as a 2016 presidential contender, is going about it in a number of ways, including an executive order that will end Medicaid coverage of hospice services as of February 1. Each year, some 1,700 less affluent residents of Louisiana have benefited from those services.
“In a strongly worded statement released on Thursday, the Louisiana-Mississippi Hospice and Palliative Care Organization’s (LMHPCO) Board of Directors stated that it was “shocked and saddened” by Gov. Jindal’s recent decision to eliminate the Medicaid hospice benefit, effective February 1, 2013.
“While the board strongly disagrees with this decision, the board is even more confused by the administration’s assertion that this action will save the state $10.6 million in FY 2014,” the board stated.
“For 10 years, the Medicaid Hospice Benefit has provided terminally ill patients and their families in Louisiana with the confidence they need to forgo expensive end-of-life hospital admissions. According to the administration, Louisiana Medicaid spent $11,430,414.14 on hospice services for some 5,189 recipients in 2012 — an average of $2,202.81 per patient.
With or without hospice, these terminally ill patients will die. The average length of stay for hospice patients in Louisiana is 72 days, with 50 percent of our patients dying within 26 days of enrollment. Without hospice, as these terminal patients decline, they will typically be transported to the nearest hospital emergency room by ambulance, and will subsequently be moved into the hospital’s intensive care units — costing the state’s Medicaid system at least four times the cost of hospice care.”
Hospice, almost by definition, is not a life-saving intervention. It is a death-management intervention. As the Louisiana hospice officials note, this alleged “savings” will almost certainly result in higher costs to taxpayers, as people who are denied professional hospice care seek medical assistance in more expensive ways.
But beyond that bottom-line analysis, there’s a question of morality and decency. Like millions of Americans, I have witnessed first-hand the value of hospice both for the terminally ill patient and for his or her loved ones. The fact that Jindal is enacting this reduction as a way to help finance the elimination of state corporate and income taxes only compounds the tragic nature of the misjudgment.

I knew three people who died in hospices. Two of them were very young—in their 30s. Anyone who wants to consign such people to dying at home has no idea of the suffering their loved ones would endure during the dying process. God bless the people who work in hospices. They need to be very strong to provide care while watching life leave their patients. If helping to run such facilities isn’t a state function, then we surely live in a jungle where nature is red in tooth and claw.

Can you or they provide some empirical evidence to prove this? I don’t see any proof of this assertion in the form of a link. As most of us know hospice is typically end of life comforting for those terminally ill. So if there is nothing that can be done for a patient dying of terminal cancer than how is it that they will be granted these new, more expensive treatments? That doesn’t make any sense. Not being snarky but please explain if you’re still on.
One other thing. There’s a good number of terminally cancer patients that can be in hospice but choose to go home to die. My gf’s dad died that way and I have a few customers at this very moment that could be in hospice but are instead spending the remainder of their days in their home. Its not unusual to want to die in the comfort of your own home.
And besides didn’t you write a column last year on the fact that in regards to end of life decisions that we are going to have to make hard decisions on the cost of treating patients for end of life care? Maybe others on here forgot about that column. Doomy didn’t. Which tells me that you are treating this case of cutting costs in end of life care in a purely political and partisan fashion.

For 1 the pt. ill not move to an ICU, that is a lot of rubbish. The hospice trade group represents, I suspect for profit centers and are thus protecting their clients–what do you expect them to say, Jay… Same old one died , superficial, partisan yellow journalism . Good example. A good number of economists proposed that the best way to handle the 07 recession was to do nothing. Why? Because the recession would have been deeper but it also would have ended more quickly and the recovery much more steeply. Instead people hollered that we needed to do something. That something has got to be done. The only thing we have to show for that is trillions more in debt and a sluggish economy.
Need another example? Take the Newtown massacre. We can spend inordinate amounts of money and time trying to do all sorts of things to ensure this doesn’t happen again. And the reality is that no matter how much we spend there really isn’t much you can do about a madman killing the legal and lawful owner of some guns and taking her guns on a rampage. But because its an emotionally charged issue people let their emotions get in the way and scream “Somethings got to be done. Somethings just got to be done”. They’re thinking with their emotions and are not thinking logically or rationally in regards to the sheer cost of doing something which ultimately probably wouldn’t make us any safer or which would make us only marginally safer. Now do you see what I’m saying?

That’s a good point and one I was kind of wondering about myself. Matter of fact my gf is a patient care tech who wanted to go to work in hospice. Why? Because it pays a lot more money. So yeah. There is some money being made in hospice care I suspect.

This will also come soon to Georgia as the Republicans attack the very weakest of the weak. In an attempt to sabotage the Affordable Care Act they are refusing to accept the Medicaid Expansion which is paid 90-100 percent by the Federal Government. Those states that expand (California, Arizona, New York, etc.) will divide the taxes collected in all 50 states by the federal government and be able to increase reimbursements to physicians and cover the large number of children who are currently uninsured. In an attempt to cover the minimum, Deal has proposed taxing the sick that are insured and hospitalized (Bed Tax). Georgians get to pay twice for an inferior product. Are we smart or what? Attack the weak – survival of the fittest.

We had hospice care for my mom in her last couple of weeks – amazing people who not only helped mom, but they also helped us to prepare for what was coming up. They really are amazing people and it’s a tremendous service.

A wee reminder–for years, whenever some rightie posts favorable comments about Hillary, and how she has “bigger balls” than Obama, and oh how they wish she’d been elected instead of Obama…
I’ve been pointing out that casual misogyny is far more socially acceptable, still, in the USA than casual racism, and if it had in fact been President Hillary, if anything the vitriol against her would be even worse that that absorbed by Obama.

2004 – US consulate in Saudi Arabia is attacked, 8 killed

2007 – grenade attack on US embassy in Greece
2008 – US embassey in Serbia is firebombed
